Claim Procedure in the event of Illness - Accident or Death

In accordance with the occurred event, the insured person, fellows or the relatives, as the case may be, have to:
  • Provide first aid to the insured person
  • promptly call the telephone number 112 in order to ask assistance of the Ambulance Service or to go to a medical assistance provider,
  • notify the insurer about the event within the term provided in the insurance contract

According to the nature of the claim, the necessary documents for opening a claim file are the following:
  • Claim declaration Form filled in by the insured person or, in case of death, by the closest relative, with regard to the circumstances in which the event occurred and the consequences thereof (original),
  • Copy of the ID card of the insured or, in case of death, of the beneficiary
  • Authenticated copy of the inheritance certificate in case such is not appointed on the policy (in case of death),
  • All existing medical documents: hospitalization note, hospital release note, medical certificate, medical documents out of which should clearly result the diagnosis of the insured and the performed medical procedures,,
  • Protocol or other documents issued by the Police, Fire Brigade , if the case may be (copy),
  • Medical documents certifying the permanent invalidity degree (authenticated copy) – if the case may be,
  • The results of the performed medical exams (e.g. X-rays, analysis certificates),
  • Medical certificate of death assessment (copy) and the death certificate issued by the City Hall (authenticated copy) – in case of death
  • Insurance policy (copy)
  • On a case by case basis, other documents or information may be required.
